Service Form US – Sample Template

Service Description:
The Service Provider shall perform the following services: consulting, support, and maintenance related to the project outlined herein, at the location specified or as agreed upon by both parties.
Service Terms:
The services shall be delivered over a period of 6 months, commencing on the date of agreement, with specific milestones and deliverables outlined in the attached schedule. Payment shall be made according to the payment plan agreed upon.
Responsibilities of the Service Provider:
The Service Provider commits to executing the services diligently and in accordance with industry standards, ensuring timely completion and quality assurance.
Governing Law:
