Equipment Description:

The equipment involved under this agreement includes a Model ABC123, serial number 987654, located at the Recipient’s listed address, or other specified equipment as agreed upon.

Release Terms:

The Owner/Provider hereby releases the equipment to the Recipient/User for the specified date(s) and acknowledges the condition of the equipment upon transfer. The Recipient accepts the equipment in its current state and assumes responsibility for proper use and return conditions.

Responsibilities:

The Recipient agrees to operate the equipment in accordance with manufacturer’s instructions and safety guidelines, and to return the equipment in the same condition as received, barring normal wear and tear.

Governing Law:

This agreement shall be governed by the laws of the State of Arizona. Any disputes shall be resolved within the jurisdiction of the Scottsdale courts.
