Procedure Description:

This waiver pertains to the tattooing procedure for the tattoo design provided by the client, scheduled to be performed at XYZ Ink Studio, or another location as agreed.

Risks and Acknowledgment:

The client acknowledges understanding the risks involved in tattooing, including but not limited to infection, allergic reactions, and scarring, and agrees to proceed at their own risk.

Consent and Release:

The client hereby releases and holds harmless XYZ Ink Studio, its artists, and staff from any liability related to the tattooing procedure, provided all instructions and aftercare advice are followed.

Medical Information:

The client has disclosed all relevant medical history, allergies, and skin conditions to the best of their knowledge, and understands that failure to do so may increase health risks.
